The Center for Respite Care is a 14-bed 24-hour facility for homeless people who are either too sick for a shelter or have been recently discharged from a hospital and need a safe place for recuperation. These patients receive basic short-term medical and nursing care as well as social services while they recover from illnesses.

REFERRAL PROCESS

  • Call Respite Staff 24/7

  • Referrals reviewed by Intake Nurse
  • M-F during business hours

  • Weekend admissions by appointment

  

Key elements of the program include:

  • Initial diagnostic medical assessment and development of a plan of care for clients upon admission;
  • Implementation of the plan of care;
  • Coordination of care with primary care physician and/or hospital staff;
  • Application for entitlement programs or coordination of plan with outside case manager;
  • Referrals to community housing and job placement services;
  • Referrals to appropriate medical agencies for ongoing care and substance abuse / mental health services as needed; and,
  • Discharge placement to stable housing or treatment facilities.
