Quick Tips for Applying for Medicaid
Step 1 - FIRST, apply for benefits (SSI/SSDI) with the Social Security Administration (SSA)

Step 2 - Submit Medicaid application in person to Job and Family Services (JFS)
Location: 222 East Central Parkway (513) 946-1000
If a person agrees, submit Authorized Representative paperwork with application

Step 3 - Collect ALL information below for face-to-face meeting at JFS
- Identification for all household members for whom you are applying or who already receive benefits (Driver's licensie,State picture ID, school reports, library card, current shot record, I-94 cards or naturalization papers, etc.)
- Verification of any income that you (or others in your home on assistance) receive or have applied for. (VA, Social security, SSI, child support, Unemployment compensation, etc)
- Verification of earned income and work expenses (last 4 weeks of pay stubs, child care expenses, etc.)
- Bank statements (savings and checking) less than 30 days old ( in general the applicant must have less than $1,500 in liquid assets to apply)
- Completed, Household Verification Form or rent receipt with your name, address, amount paid and landlord's name and phone number, or mortgage book
- If employed, a completed Employment verification form and most recent pay stub or W2 forms.
- If emplyment was terminated in the last 6 months to 1 year, get proof of termination letter from last employer.
- Awards letter from Social security Administration (or receipt for SSA application)
- Current Utility bills (DUKE energy, water, trash, phone)
- If applicant owns a car, take the title to the appointment. Verification of amount owed on all cars.
- Verification of any assets (life insurance policies including face ans cash surrender values, burial plots-need title, stocks)
- If pregnant, proof of pregnancy signed by doctor
Step 4 - Receive notification of face-to-face appointment at JFS

If it has been more than 30 days since you applied, call 946-1000 and speak to the application desk regarding status of application.
If you are an Authorized Rep for the applicant, confirm that applicant received notification also, determine if client will also attend appoitnment, arrange transportation if necessary.
Step 5 - Attend Intake Appointment
Take all inforamtion collected in step #3.

Step 6 - Submit any information deemed "missing"
Deliver in person and get receipt
OR fax and call to confirm receipt
Step 7 - Receive "Awards Letter" - notice of approval or denial
Determine coverage awarded to the applicant
