Next of Kin Registration Process In Case of Emergency
(Eligible if you have Ohio drive License, Temporary Permit or State ID)
Ohio is one of the first states to make a next of kin registration process available through its BMV. Any holder of a valid Ohio driver license, temporary permit or state ID card will now have the option of providing the name and information of a contact person they wish to be notified in the event the individual is involved in a crash or emergency and is otherwise unable to communicate with the contact person. This secure database will only be accessible to BMV employees and law enforcement officials.
Ohioans can now provide next of kin information for up to two contacts through any BMV deputy registrar or online at
https://www.dps.state.oh.us/netsys/netdb/ENGLISH/LKUPX.asp. Those under the age of 18 are required to provide a parent or guardian for the primary contact. For more information, visit www.bmv.ohio.gov.

Effective immediately St. Vincent de Paul will assist clients with ID and Birth Certificates the first Thursday of each month only.

Colorado - Valid Driver's license, State I.D. or Passport
Connecticut - Photocopy of picture I.D.
Delaware - Valid Driver's license, State I.D. or work I.D.
Washington D.C. - Photocopy of Picture I.D.
Forida - Photocopy of Valid Driver's license, State I.D., Passport or Military I.D
Idaho - Photocopy of Valid Driver's license, State I.D., Passport or Military I.D

Louisiana - State issue Driver's license (can be expired as long as it shows no signs of tampering), State issued Picture I.D.. (same exception as Driver's license), current US military I.D> with potograph or a high school year book as log as the picture clearly identifies the person.
Maryland - Photocopy of Valid Driver's license, State I.d. Passport or Military I.D.
Minnesota - Application must be notarized or signed in the presense of a registrar as well as a Valid Government Issued I.D.
Nebraska - Photocopy of Valid government Issued I.D
Nevada - Photocopy of a picture I.D.
New Jersey - Photocopy of Valid Driver's license, State Id, Passport or Military ID
New Mexico - Photocopy of Valid government issued picture ID
New York State - Photocopy of Valid Driver's license, State ID, Passport or Work ID
New York City - Valid Photo Identification Defined: Identification (ID) with a photograph of the bearer that has the signature of the bearer. ID must be issued by an officially recognized organization or agency and includes the following types of ID: Driver's license, Employment ID, Government ID, Social Services ID, and a Passport.
Oklahoma - a photocopy of valid, legal photo ID is required from the applicant or the individual representing the applicant for the issuance of a irth, death or stillbirth certificate.
Pensylvania - State issued driver's license or non-driver photo ID with requestor's current address or passport. If possible, enlarge photo ID on copier by at least 150%
Rhode Island - Photocopy of Valid Government issued picture ID
Tennessee - a photocopy of a Government issued state ID
Texas - photocpy of valid government issued Picture ID
Utah - Photocopy of vlid government issued picture ID
Virginia - listed on application (long list of primary documents)
If client does not have a Valid Government issued picture I.D. there are only two ways obtaining Birth Certificate and Identification Documents.
1. If the client has a Government issued picture I.D. and it expired less than 6 months ago, a local BMV will reissue one without a birth certificate.
2. If the client has appeared before the courts or been arrested in Hamilton County, they can get a certifies copy signed by a judge with their last court date from room 315 at the Court House. This coupled with a SS card or printout can produce a State I.D. from the Court Street BMV.
