Healthy Start & Healthy Familes Medicaid Healthy Start provides complete health care for fids from birth to age 18 and pregnant women up to 60 days after the baby is born. Your family size and income will determine if you qualify for these programs. There is no limit on the resources or assets you own to qualify. If an asset creates income however, it is counted to determine if you qualify.

Having health insurance from another group does not keep parents and most children from being on Medicaid. Children in families with incomecloser to the upper income limit cannot have both Medicaid and similar private health insurance. Medicaid pays for doctor visits, medication, hospital care, vision and dental care, immunizations, mental health, prenatal care, and more.
How to apply for these programs: Call the Hotline at 1-800-324-8680, 7 days a week. Staff can help you fill out forms. You can also go to your local office of Job and Family Services, but no face-to-face interview is required. Mail, fax or bring in you application the the Department of Job and amily Services at 222 East Central Parkway. Fax umber is 513-946-7603. Attach your pay stub as proof of income. If need be, include items that prove pregnancy, or other health coverage. You must reapply for children on Healthy Start every 12 months. Families must reapply every 6 months. Anyone applying for Medicaid must give one-time proof that you are a U.S. citizen. A document, such as a U.S. passport, can prove you are a citizen. If you do not have that type of paper, you will need proof of where you were bor and proof of your identity.
