CareSource Medicaid HMO - Transportation Assistance
Transportation can be provided for medical appointments, if needed. CareSource covers up to 15
round-trip visits (30 one-way trips) per member per 12-month period to any health care appointment. If you must travel 30 miles or more from your home to receive covered health care services, CareSource will provide transportation to and from the provider’s office. Please contact Member Services at 1-800-993-0780 (TTY: 1-800-750-0750) for assistance. You must call to arrange a ride at least 48 hours (2 business days) before your appointment.
Transportation is provided by private shuttle van. Please see the Transportation Services flier you received in your CareSource membership kit.
