What healthcare services are provided at Crossroad?
Crossroad provides physical, mental and spiritual health care for adults and children including check-ups immunizations, management of acute and chronic health problems, and mental health counseling.
Services are provided by both staff and volunteers.

How much does it cost to see a doctor at Crossroad?
No one is turned away for their inability to pay for services. Charges are based upon a sliding fee scale. Medicaid, Medicare and most commercial insurances are accepted.

**Services for the The Gift of Sight Program & Prevent Blindness Program
are available at the three Wellness Program sites listed on the next page.
The Gift of Sight (Wellness Program) – The purpose of this program is to provide free prescription eyeglasses. Individuals eligible to receive this service must be an Ohio resident, 19yrs or older (school age children have a Gift of Sight Program available through their public school), low income (less than 200% of Federal Poverty Level), have no vision health care insurance coverage, have a prescription for eye glasses less than two-years old, has not received eye glasses through the Gift of Sight Program within the last year and has a current photo identification. Persons wishing to apply must come to one of the three Wellness Program Sites listed on the next page (with requested documentation), fill out form, and select preferred location to get eye glasses.
Prevent Blindness (Wellness Program) – The purpose of this program is to provide a comprehensive vision care program for medically indigent and homeless individuals to include vision screening, eye examinations, and procedures as needed at no cost to those who qualify. Individuals must be 19yrs or older, low income (less than 200% of Federal Poverty Level), have no vision health care insurance coverage, at risk for blindness (has glaucoma, or diabetes or a family history of either or both). Individuals must meet the previously stated guidelines and fill out a Prevent Blindness application at one of the three Wellness Sites listed on the next page. The Wellness Program will fax the form to Prevent Blindness Ohio. Once notification is received from Prevent Blindness (via fax) regarding the individual’s acceptance into the program and an appointment for testing, the Wellness Program will call the client to let him/her know the screening date and place and mail the same information along with a client identification card.
Discounted Vision Test (Wellness Program) – For those who can afford $40.00 and cannot wait the 3-5 months it may take to get an appointment at UC Eye Clinic. Having Medicare or Medicaid is OK just no vision policy and low income are the qualifiers.
